HIV Care & Treatment Programs
The Early Intervention Services Program and HIV Case Management Program provide a wide range of support and care services to persons living with HIV. The goal of these programs is to ensure that individuals with HIV infection have access to skilled, comprehensive outpatient care and support services, therby improving their health and interrupting the transmission of HIV.
Available Services
Program services include:
o HIV specialty and primary medical care
o Medical case management
o Health education and risk reduction education
o Access to medications
o Support services such as food, housing assistance, and medical transportation
Services are available to HIV positive individuals residing in Lassen County.
Purpose & Strategy
The purpose of the programs is to provide HIV health care providers of the five-county region with up-to-date information and resources related to HIV intervention services and case management programs through maintaining program manuals and providing easy access to information and program-related forms.
