Environmental Health Complaint Form

Lassen County Environmental Health is the enforcement agency responsible for the inspection of various types of businesses from restaurants, swimming pools, water wells, septic tanks and landfills.

You can use this form to report a problem or notify us about public health conditions such as:

  • Problems at a restaurant or food market
  • Sewage or wastewater discharge at a property
  • Accumulated trash or debris at a property
  • Public recreational waters, pools/spas, and beaches
  • Lead exposure hazards (from lead-based paints, etc.)
  • Problems at a body art facility
  • Organized camps
  • Improperly managed hazardous or medical waste

We do not investigate: mold complaints, building violations, hotel/motel conditions (unless it is about an included food facility), or public housing.

Lassen County Environmental Health will address your concerns as soon as possible, and you will be contacted with the results of our investigation. All complaints are handled confidentially. Although anonymous reports are accepted, we request your contact information so that we may contact you if we need additional information to properly address your complaint. In the event that staff is not able to investigate your complaint because of jurisdiction issues or lack of licensed professionals we will follow up with you (if contact info is provided) and provide resources and/or contacts if possible.
